Please note that prior to starting your divorce, you should gather all of the following information, as it will all be necessary in order to properly process your divorce action:

For both spouses: Name, address, date of birth, state of birth, telephone number, social security numbers, race, the number of previous marriages, number of previous marriages that ended in death, number of previous marriages that ended in divorce / annulment, last level of education, employer name, employer address and employer contact information (if known), health insurance provider name and address.

For the marriage: the date and place of the marriage, the date of the parties' physical separation, the parties' last shared address, the number of children born from the marriage.

If there are minor children born from the marriage: the name(s), date(s) of birth, and social security number(s) of the child (children), and the parent taking custody of the minor children. For child support purposes, the parties' salary information (prior year's gross income amount) must be provided, as well as costs for childcare and uninsured medical expenses. Medical insurance information must be provided for the party providing the children with health insurance, if any.
